Stefan Hajnoczi 1056a1c2d5 [prefix] Remove unsupported ELF prefices ...

The .elf, .elfd, .lmelf, and .lmelfd prefices were brought over from
legacy Etherboot and they do not build in gPXE.  This patch removes the
ELF prefices.
